Rules of the marketing promotion "50% discount for every 20th client"

I. Introductory provisions

1. The organizer of the marketing event called "50% discount for every 20th client" (hereinafter referred to as "Event") is the company Svět rovnátek s.r.o., with its registered office at Nad Vršovskou hora 1423/10, Michle, 101 00 Prague 10, ID No.: 186 27 218, registered in the Commercial Register maintained by the Municipal Court in Prague, Section C, Insert 1857 (hereinafter referred to as "Organizer").

2. The Promoter issues these Rules of the Promotion which govern the terms and conditions of the Promotion (the "Rules"). By participating in the Promotion, the Participant agrees to these Rules and undertakes to comply with them.

II. Duration of the Promotion

The Promotion runs from 19 May 2025 to 9 June 2025 (the "Promotion Period").

III. Conditions for inclusion in the Action

1. The Promotion will include all natural persons with a delivery address in the Czech Republic who, during the Promotion Period, enter into a Health Care Contract with the Promoter themselves or through their legal representative (in the case of minors), the subject of which is treatment with invisible braces, and who also commence treatment during the Promotion Period, i.e. fulfill the terms of the concluded contract (hereinafter referred to as the "Participant").
   

2. Each Participant may participate only once.
   

3. The winner will be determined by the order in which the Participant signs the Care and Health Contract.
   

4. Every 20th client who enters treatment during the Event Period will receive a 50% discount on orthodontic treatment provided by the Promoter.

IV. Prizes and determination of winners

1. The prizes in the Promotion are 50% off invisible braces (" Prize").
   

2. The winner will be determined by the order in which the Participant signs the Care and Health Contract. Every 20th client who enters treatment during the Promotion Period, i.e. from 19 May to 9 June 2025, will receive a 50% discount on orthodontic treatment provided by the Promoter.

V. Notification of Winning and Delivery of Winnings

1. Each Winner will be contacted by the Promoter by email within 5 working days of the end of the Event to the email address provided as the contact details of the Promoter (" Email"). The Winner must respond to the email from the Promoter within 5 working days of receipt of the email to indicate whether they accept or reject the Prize. If he/she fails to respond, such Winner will be eliminated.
   

VI. Notification of the processing of personal data

1. The Organiser is the controller of the personal data of the Participants of the Event. The personal data of the Event Participants will be processed in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016, General Data Protection Regulation (hereinafter referred to as "GDPR") and Act No. 110/2019 Coll., on the processing of personal data, as amended.
   

2. The Organiser will process the personal data of the Participants for the following purposes:

• for the purposes of the Promotion, for the purpose of evaluating the Promotion, informing the Promotion Participant of the award and delivery of the Prize;
• the provision of health services (orthodontic treatment) on the basis of the Health Care Contract;
  

3. The Organiser will process the Participant's personal data in the following scope:

• first name + surname, birth number + date of birth;
• contact details (address, email + phone number);
• the insurance company with which the Participant has his/her health insurance;
• an anamnestic questionnaire (includes information on any chronic or infectious diseases, bad habits, type of toothbrush used, etc.);
• medical records from the Participant's visits to the Organiser (photo documentation and X-rays, digital scans of teeth).

4. The recipients of the Participant's personal data will be authorized employees of the Organizer as well as Align Technology Inc., 2820 Orchard, Parkway San Jose, California 95134, phone 408-407-1000, manufacturer of invisible braces.

5. The Participant's personal data will be processed for the period of time stipulated by the relevant legal regulations.

6. Each Participant has the right to access his/her personal data and to correct, delete, restrict their processing, the right to their portability, the right to object to their processing, as well as the right to withdraw consent to their processing at any time. The participant has the right to lodge a complaint with a supervisory authority if he/she considers that the processing of his/her personal data violates the provisions of generally binding legal regulations.

7. The Entrant's personal data will be processed in accordance with the Organiser's Personal Data Processing Rules. These rules are published at www.svetrovnatek.cz

VII. Final Provisions

1. Excluded from the Event are the Organiser's employees or persons in other similar relationship to the Organiser and persons close to such persons within the meaning of Section 22 of Act No. 89/2012 Coll., Civil Code, as amended.
   

2. The Participant is not legally entitled to the Prize by participating in the Promotion. The Prize cannot be claimed in court.

3. The prize cannot be paid in cash or exchanged for any other monetary or non-monetary consideration.

4. The Winner is fully responsible for meeting any tax obligations arising from the tax regulations of the Czech Republic that may be associated with the acceptance of the Prize.

5. The Promoter shall not be liable for any defects or damages arising in connection with the implementation and use of the Prize.

6. The Promoter has the right to change the terms and conditions of the Promotion at any time, including during the course of the Promotion, to change the duration of the Promotion, i.e. in particular to extend, interrupt or terminate the Promotion early. Any claims or objections shall be finally decided by the Promoter.

7. The Promoter reserves the right to exclude from the Event, without further delay, Participants who are found or reasonably suspected of violating these Rules or failing to comply with the conditions for participation in the Event or who share vulgar, offensive, racist or otherwise inappropriate statements or posts that are contrary to good manners or damage the reputation of the Promoter, without any compensation.

8. The Promoter reserves the right to change the Prize if it is unable to deliver the original Prize to the Winners for reasons beyond its control.

9. The Promoter reserves the right to change these Rules. Any change to the Rules will be announced by the Promoter in the same manner as the Promotion was announced and will be effective from the time of publication.
